Build a form or quiz with Forms

Create a form from New in Drive. Start blank or from a template: contact form, event RSVP, customer feedback, job application, quiz, order form or employee survey.

• Click a question to edit it. Choose its type from the menu next to the title: short answer, paragraph, multiple choice, checkboxes, dropdown, yes or no, linear scale, rating, grids, date, time, email, number, phone, website or file upload.
• Use the buttons beside the form to add a question, a title and text, or a section. Drag the handle at the top of a card to move it, and the handle beside an option to reorder options.
• The More menu on a question adds a description, an image, response validation, Show only if... (hide a question unless an earlier answer matches) and Go to section based on answer.
• Under each section, After section decides where people go next, including straight to submitting.
• Theme sets the colour, a header image, the font and whether people see a light or dark page.
• Turn on Make this a quiz in Settings to give questions points and an answer key. Scores are graded automatically and shown straight away or when you release them.
• Preview shows exactly what people will see, on a phone or a computer, and updates as you edit.

Other people can build the form with you: use Share to add editors. You see their faces on the card they are working on, and comments can be left on any question.
